1. All * fields are mandatory. Registrant must select (i) subscriber; or (ii) member of relevant supporting organization (with membership number) in the field of “Institute Affiliation” to enjoy discount. 2. Registration process:
a) An acknowledgement will be sent to the registrant's email address upon receiving your registration form.
b) Please print the acknowledgement email and send it together with your payment (please make cheque payable to “Practising Governance Limited”) to our office at 17/F, Hong Kong Jewellery Building, 178-180 Queen’s Road Central, Hong Kong before 25 November 2016 . If you use the same contact email address to register for a number of persons, please make out one cheque for the total payment.
c) A formal attendance confirmation will be sent to the registered participant’s email address after receiving your cheque. Please call us on 2509 3928 if you do not receive our confirmation 3 working days before the event. (We are aware of instances where our emails have been blocked by companies’ IT systems).
